To ensure that your PhilHealth number is captured correctly, you are required to submit a proof of your PhilHealth record by submitting any of
the following:
o PhilHealth ID
o Processed M1a Form (PhilHealth Number Application Form)
o MDR (Member’s Data Record)
o Print-out of any PhilHealth-related document
If you are not sure if your previous employer was able to obtain a PhilHealth Number for you, you can inquire in any PhilHealth Office. You may check the branch nearest you through www.philhealth.gov.ph or call 637-9999.
If you do not have a Philhealth number yet, you may request a copy of your ER2 Form from your previous employer. Proceed to any PhilHealth Office, present the ER2 form and request for a printout of your Member's Data Record. Submit a copy of your MDR on your Day 1.
